Before his transfer, he also played in an AAU team includingf Dwight Howard and Randolph Morris. His senior year at Oak Hill was strong and he helped the team to an undefeated 38-0 record. He was considered the number one small forward by Rivals.com but opted to forgo college basketball to enter the NBA draft.

Smith joined the 2004 NBA draft and was selected as the 17th overall pick by the Atlanta Hawks. He performed well during his rookie year and eventually became the Slam Dunk Contest champion, earning three perfect scores. He also became well known for his defense, and had reached the 500 block mark during 2007, the youngest player to do so in the NBA. He continued this record into 2010 with 1,000 blocked shots in his career, still the youngest player to achieve this target.

Eventually, his contract with Atlanta ended after the 2012 season, and for 2013 he signed with the Detroit Pistons on a four-year $54 million contract that raised his net worth dramatically. He continued to perform well, peaking at a season high 24 points against the Dallas Mavericks. However, during the 2014 season, he was then waived by the Pistons as his percentages and averages began declining. He then signed with the Houston Rockets, suddenly showing a second wind of performances. He was moved to the starting line-up but his performance dipped soon after, and he slipped back to the bench. Still. he continued to show his prowess in the 2015 NBA Playoffs against the Dallas Mavericks and the Los Angeles Clippers.
